Beginning Tuesday, October 12, all high school students must complete a daily symptom check before they enter the school building.

Students will submit their daily symptom checks through this Google Form.

Parents may submit a symptom check on behalf of their child using this separate Google Form. The confirmation of completion will be sent to whichever email is inputted in the form. If it is sent to the parent's email, they will need to forward that to their child. 

On Tuesday, students will be required to show a screenshot of their completed form at building entrances. Students who have not completed their daily symptom check will be asked to submit their form before they can attend classes. Hard copies of the daily symptom check will also be available onsite.

If a student is symptomatic at school, they will be required to submit a negative COVID-19 test before returning to attend classes.
